What evidence can I provide to show Supervision?

Supervision evidence can be provided in the following forms: 

  • Letter from current or past supervisors on company letterhead stating the total number of hours of supervision received in the time frame you were seeing them either in private practice or in the workplace.
  • Signed workplace Logbooks or supervision records showing clearly the individual hours provided (not supervision notes, as they are confidential)

We can not accept statutory declarations for Supervision hours.
